During the dialogue in front of the SMOKER ON THE BALCONY's apartment #28 (when you find out that he isn't at home) Kim Kitsuragi's little notebook is incorrectly referred to as "black", when just moments earlier, while interviewing the SMOKER ON THE BALCONY it's described as "blue" (see the description on the right) instead.
Which, also according to previous other instances of the notebook's colour being described should indeed be the correct one.

I am having a problem where none of my saves load properly, not even really old ones, the main menu UI comes up along with either a black or scrambled picture instead. I was at the island when this started happening, the game was fine before that.

The ingredients combination-feature in the 'MEDICINAL PURPOSES OF THE PALE' book doesn't work properly, at least for further attempts (incl. certain ingredients) past the initial one you get +5 XP from (first snippet in the upper left corner).

The issue probably could be described in general as: certain ingredient combinations and their corresponding expected results being ignored and replaced with/defaulted to another, completely unrelated outcome, if that makes any sense.

For instance, if you start off with anything that includes rue as the base ingredient and in the following attempt mix something together with mint it properly shows the corresponding outcome ("combination of mint and birch bark") but if the first attempt involved mint and the next one rue as the base ingredient it does not result in, for example "rue and aloe vera make this poultice..." but the description of the concoction from the previous attempt ("combination of mint and birch bark") instead.

In the illustrated case (see the screenshot collage) - once you do include either ginger (as the 1st base ingredient) and/or whiskey (as the "appropriate vehicle") it locks/incorrectly defaults the final response presenting you with the result to:
In this particular case the "julep of mint" one, with the introduction of whiskey (third snippet upper right corner).
From there on any other ingredient combination that doesn't include ginger results in the "julep of mint"-result.

Once ginger is brought into the mix (fifth snippet middle bottom) the "julep of mint"-result is no longer brought up and gets replaced by the "tincture of whiskey and ginger"-response as the new default outcome to any further ingredient combination from there on.
