I run XP SP3 nvidia 9600 GSO. Had widescreen mod installed, kept going back to desktop when entering a building. I deleted all my save games, re-installed widescreen to same resolution as my desktop, and now have virtually no problems at all. When entering a building it reverts to top left of my screen, but can manage with that. Sometimes the area of map not yet discovered is very blocky, but select map and then back again and all is well. My wife also runs it ok on her PC, again with nvidia, 8600 gt i think, again works well.
Perhaps we are just lucky!
For ATI cards try running your 3D settings on optimal performance…
I've been having crashing issues just fiddling with the radial menu and trying to intiate dialog with my party members.
I was doing fine, but it crashes every time I try to enter the house of the woman who teaches you how to be a mage in Ragpicker's Square. Reload, crash. Reload, crash. Reload, crash. I installed all the mods that the mod guide thread here recommended.

Edit: The most recent time I tried it I got this error message:

An Assertion failed in F:\Torment\Source\baldur\CVisibility.cpp at line number 1264.

For Vista/7 users, try disabling UAC and/or installing the game outside Program Files.

I hate to say this but I reproduced the bug using the Gog version :/ Exactly what happens, it tries to boot up the next section and it dumps me to the desktop. It was leaving the Mortuary through the portal. However, I will say that I wasn't able to reproduce it thereafter. I don't know what to say.
Having the same issue as well. Windows 7 with GoG-Planescape installed, along with the mods: The Bigg's widescreen mod (At 1920x1080), Ghostdog's UI, and then all inclusive fixpack by Qwinn.

What's odd is that I didn't have this problem with an older version of the game running at 1440x900 resolution.

Also the crashing happens at specific points. For example, attempting to exit the Witch's hut after healing almost always ends in a crash. Right now, trying to navigate the museum in the Lower Wards (I believe is the area) causes crashing in most of the southern area. I'm a fair few hours in, so I don't want to restart, but it's not really looking up.

Looking at these threads, I've tried: playing the game in another location on the hard drive (Not program files), running with Admin rights, and turning UAC off altogether.

Edit: reinstalling PST and then reinstalling the mods seems to have worked. I've gone up to the the witch, where if I healed, it always crashed as I left the hut. It hasn't crashed yet, so I am feeling comfortable playing some more.

Edit edit: Nevermind, now it's crashing in areas it hadn't crashed in before (Office of Vermin something something) and a random Hive house.

This is getting a bit annoying. Every time I enter a building it CTD's about 50% of the time. Thankfully the autosaves can be used but still, constantly restarting the game is putting me off.
Hopping in on the crash bandwagon.. I had zero crashes before, but instantly when I reached the lower ward the game started crashing randomly when it loads a new area. Very annoying.

Having this problem on Vista 64 bit, widescreen and modfixes installed. Its odd, because sometimes it'll crash at my first attempt to enter/leave, and sometimes it'll let me play 2-3 hours with no issues. I'd like a fix for this though.

Same here. 100% reproducable if trying to enter The Apothecary. WinXP, NVidia.
Is torment.exe still running after a crash to desktop?
