Why is that wrong? As far as I'm concerned, the fact that you don't see everything in spite of finishing the story is one of the most compelling things about storytelling in videogames. Not to mention that old games did it too (just as most of your points, by the way)
Frankly, I think DRM pretty much takes the cake in being the worst pest.
YES!!!!! on the tutorials. I am blown away how many games I play and they are literally telling me to click somewhere to move my guy. Usually it's Tutorial 5 or 6 before I learn anything, and they usually only unlock 1 at a time.

Tutorial 1: Click to move your man. Move him over the bridge.
Tutorial 2: Double click to run. Run him over the bridge.
Tutorial 3: Never walk. Just push CAPS LOCK because running is the only thing you want to do. Run across the bridge again.
Tutorial 4: Click on the lever to raise and lower the bridge. Now run across the bridge.
Tutorial 5: Right-click and drag to change your facing. Face north while standing on the bridge.
Tutorial 6: Simply hold shift while pressing I to open your inventory, select the X-95 T-Class Rocket launcher, hold H while repeatedly tapping Q or E, followed by a double mouse click to launch yourself into the air, grow wings, and fly. Walking is only used in the tutorial. Enjoy the rest of the game!

I think what OP doesn't like is having to replay those parts of the game which have been seen. I kind of agree, but I guess the game should be interesting enough to pull you through a 2nd playthrough w/o boring you, the Witcher 2 was like that.

Possible. All the games mentioned in the OP are quite replayable tho and losing the possibility of affecting the story in different ways would take some of the appeal (just as RPG elements not helping out in reaching certain areas etc.)

Well, I'd give some malus in dexterity, strength, stamina and charisma for mens.
This is stuff that affects you heavily in combat (also in RL martial arts), unlike taxwork, dishes, etc.. :P

PS: There would always be the possibility to play a male character for you. :P

I guess this is related to point 1, but the constant push to try and keep people online as much as possible. If a customer logs in once, gets their game, logs out and plays it at their leisure you can't keep pushing DLC, the sale of the hour/day/week, or whatever other gimmick is popular at the moment to them.
4-6 don't bug me (especially not 5. I LIKE #5), as long as 4 and 6 aren't too overboard.

You forgot DRM and Achievements from your list.

You also forgot freemium/f2p/freetostart games, but they don't function without DRM, so they're included.

Okay, then women should get unlimited berserker with extra damage points when they are PMS'g. Because when I am PMS'g I get mean, real mean. Cliché but there it is. What I'm saying is that when you start trying to get all the real life situations in a game it can get overburdened and boring. Taxwork, dishes, taking out the garbage and Real Life in general seriously cuts into my gaming time as it is, LOL, I don't want to do it in a game too.

Yes in RL the things you say would have a serious affect on combat.
However, in RL you are not going to fight Orcs, gargoyles and you are not going level up every time you beat up a bully.
You are not going to win a fight carrying just a dagger against a gun because you have more hit points then opponent.
You are not going eat, sleep and live in 50 pounds of armor and carry all your worldly goods (another 50 or 60 pounds) around on your back without some affect on your ability to fight.

1. DLC spam

DLC per se is not a bad thing. If you do it right, people can like it. What I talk about is the DLC spam that we saw on several modern titles like Civilization 5, Borderlands 2, etc.

There is the main game, and then there is a plethora of DLC, some of which is story DLC, some of which is not. My only advise for such games is to wait until all DLC is finally released and then buy it all together. How do you know all DLC is released? Well, either there is a Gold version, the next game in the series is out, or the developer is bankrupt.

2. Pretending everyone is an idiot

If you want to make a tutorial, it is fine. But don't force us through the tutorial. If I have to play some new game and can't figure out the controls, let me see the controls at any time. If I don't understand the mechanics, let me do a tutorial or explain it with some ingame text. Do not force me through the tutorial, if I don't need it.

3. Fail-proof difficulty or too high difficulty

Let people suffer if they want to. Make a difficulty that is always fair but challenging. Don't make games too easy or too hard. It is annoying if you can essentially breeze through a game unless that game is of the kind to breeze through, but also annoying if the game leaves you no chance at all. Let us set the difficulty we want, please. If you want to finish your RPG in 20 hours, so be it, but let me fight against the ultra-hard enemies for 100 hours!

Seen in: Many titles, gladly avoided also by many. As an example, Last Dream lets you set the difficulty in several levels and the highest IS a challenge while all others are more suitable for gamers that want to progress without too much grind.

4. Multiple playthroughs enforced to unlock game options

Do you like it if you play through a game and only after you finished it, the highest difficulty is unlocked? No? I also don't! Let me choose the difficulty I want, etc.
If you want multiple playthroughs for achievements, that is a different thing but also not nice.

6. Missable collectibles or collectibles distracting from the gameplay

What is the reason why I need to find intel in FPS games? When the setting fits and nothing is missable, I am fine with collectibles - in Far Cry 3, GTA, etc. they enhance the fun - but there are genres where collectibles just don't fit. Also, it is NOT fun for me to have to use a guide all the time just to make sure I miss nothing, instead of being able to play.

Seen in: Spec Ops: The Line, Call of Duty: Modern Warfare 2, Bioshock Infinite, etc.
Number burns me up too. I hate DLC when I have to pay extra for it.

Touché, my gf has PMS atm, although she is more the depression than berserker type, but I'd aggree to a berserker rage bonus prior to the mens-mali. :D General strength malus for female characters was my main concern though, you can make it harder for females to level up strength and easier to level up charisma or something like that. I would finally play a female character out of my own free will if it creates a different gaming experience, probably a higher difficulty now and then. :P

Well, there are RPGs that make you organize your stuff and body functions/limitations.
The great </span> comes to mind, from the maker of <span class="bold">[url=http://www.gog.com/game/driftmoon]Driftmoon. I think Robinson's Requiem got that too, both games are rather hard because of that, but great.

Another pest of modern gaming:
Regularly forcing to upgrade your PCs hardware and OS like Win7 with another online activation.
This will get worse now that support of WinXP is gone.
