- obnoxious tutorials that can't be skipped.
- dialogue and elements that practically shove in your face 'hey, we're cool with diversity, sexuality, and all that noise!' again, and again.... I fucking get it alright!
- Cheese tactics in MP games. Kinda like in Star Wars Battlefront [EA]
- Choices that have zero real impact besides a change in dialogue (passive NPC talk, a line in a conversation).
- no difficulty setting (some games become incredibly easy and you just can't change it)
- difficulty settings done poorly (huge gaps between settings from 'hey, this is doable but I want it a tad harder; to 'holy shit I need to be MLG all day to even attempt it'.
Weighted items in games that give you a very limited inventory space. :/
I love collecting stuff and Im too lazy to sort out stuff in my inventory now and then.
As an example: You have a couple books that take up 7 units of space while you only have 70 units total.
So annoying... >:|

Oh, durability is annoying as well. Repairing your guns/ blades or whatever after each fight is tedious and no fun at all., especially if you forgot to repair your stuff and it breaks during a fight....
NPCs doing stupid thinks.
I played GTA Vice City in the last days and while I'm driving and come close to passengers they make a jump trying to evade me. But very often they don't jump away. Instead they jump direct in front of my car, which is annoying esspcially, if it was a policeman.

I also don't like a bad saving system.
