I finally got it working decently on Win7 64-bit by forcing it to use only one core with RadeonPro. I think you can use other programs to do something similar, or do it in the Task Manager somehow. That definitely improved the framerate, but it still drops badly in city areas. I think it has some sort of bad vsync that wants to be either 30 or 60fps. Trying to force changes either didn't work or crashed the game.
The game is unfortunately locked at 1024x768, and afaik there is no way to increase that. Note that the game assets are 2D with 3D models/characters on top of them. So to go to a higher resolution you would need to expand the field of view rather than blow up the image. Diablo II has a mod to do that, I wish Sacred did as well.
The game looks fantastic on my old CRT monitor, I feel like maybe it needs some scanlines etc. to really 'pop'. However, I can't get SweetFX or anything else to work with the game, so adding those, or some post-processing AA, is impossible as far as I can tell.
The game just seems very... "locked"... if that makes any sense. Trying to change anything externally seems to result in crashes or have no effect. I really wish GoG would take another look at updating this title for modern systems.
