QWEEDDYZ: I didnt plwy newest one. FOr original and almost all sourceports incl Q2RTX there is such issues. Some enemies sometimes struck in rooftops where they spaw and should drop at your back ("dogs" and medics).
There is one enemy with gauss gun put to box that never opened (wupposed to be "blow" with enemy revealed but never have such script).
This is issue on Hard (and Hard+ as they are same) difficulties only.
Original maps should be patched-rewrited for Hard mode. Newest version include such patches. Maybe still devs forget something or didnt test Hard mode.
YamagiQ2 also come with SP map patches as additional files in base2/maps/fixes subfolder or something like that. Not touching original maps in pak archive but load dynamic fixes. Someting like that shoupd be in Enhanced.
Q2RTX have no such fixes (v.1.5).
Are you sure you didnt find last enemies? It can be medic's rised or spawned units backward from your position. Spawn is new feature (as i heard it).
