Seems that the game flickers on my configuration, I was sarching for a solution here: http://www.outcast-thegame.com/support/patchindex.htm
and they say to try to enable v-sync, only that the game DOESN'T have a v-sync feature, is forcing it on the video card panel the only viable solution?
They also say trying pressing ctrl-esc and then refocus the game window but it doesn't work at all

In another thread someone said to turn off Catalyst A.I. if you're running on a ATi Radeon graphics card. I hope that helps.

How about this same problem on an Nividia card?
I've got the same problem only I'm also on an Nvidia card and I've only got one (so no SLI)
I'm on Windows 7 64 Home and my card is a GTX 260 if that helps

Cool so not just me then. Windows 7 32 Home here, with a GT 220. Tried turning off Vsync on the Nvidia panel and then the game wouldn't even load! A shame because apart from that runs pretty great. But can't play with flickering, hurts eyes!
UPDATE: Updated my Nvidia graphics driver. Heart jumped outta my throat at one point because destop background displayed and nothing else. Put PC into sleep by pressing power button, brought it back awake and everything was fine, driver updated. Phew. Still... same problem with Outcast! So latest drivers don't help...
I also found the GOG.com support on the issue here:

This makes it even more frustrating, as it is a known issue, yet my PC is neither a Radeon card nor uses multiple graphics cards. Totally stumped!

I have the same issue with a single Nvidia card. I've tried to deactivate all the settings on the Nvidia Control Panel and to turn VSync both on and off but nothing has worked.

The control panel isn't going to help at all. Nvidia and ATi control panel options only influence Direct3D and OpenGL settings, and Outcast is completely rendered in software mode.

I knew that Outcast is completely CPU rendered, but I tried driven by despair. XD
Just for reference, i'm on Win7 64 bit, GTX 295 with 197.13 drivers and no issues here.
You guys tried disabling visual themes on the shortcut ?

It is already ticked by default AND greyed out on the shortcut, with Windows changing to Windows Basic once the game is launched automatically. Everything is perfect, the sound, the launcher, the controls... the only problem is this constant, spaced out flickering. Grr!

Ah, i see. I'm sorry to say i don't know what else to suggest.
You say that option is greyed out by deafult ? I don't think it makes any difference but i wonder why it's not greyed out ay my end.

I have the same issue. Its driving me crazy. although I found it runs pretty smoothly unless you rotate the camera
