mistermumbles: Well, Trackmania comes to mind. Can't think of much else than that though.
Trackmania does not require loading to go into your track, but usually you don't have a live play going while you edit.

I actually think UT3 tools supports something like this, not sure, never been a mapmaker except a few Trackmania tracks, but think I've seen tutorials for an editor (I think it was a UT tutorial) which showed the game running in a window beside your editor which you could play as soon as you'd made a small change.
I can't seem to access the editor. It just goes to a black screen and I have to cold boot to restore computer functions. No idea what's wrong.
I don't think i could come up with anything different to what is already there. But a self made game expansion is a novelty.

Perhaps i may try it some day.
I didn't want to download the entire installer again (bandwidth ain't free), but I got the patch directly from Almost Human's website and it worked just fine.
Started work on a dungeon last Sunday. After finishing the first floor, I'm already about to call it quits. To be able to do anything remotely interesting during dungeon building, you have to be good with coding (lua). Okay, it's not a horribly complex scripting language, but (as a complete novice) when I have to browse through Almost Human's forum's modding section every time I want to do something more than just "hit switch/pressure plate here" it becomes very tedious, very quickly.

My introductory dungeon level took me the better part of a day to complete, and it's all rather simple still. =/ I may try to pick it up again eventually, but I've put the project on hold for now.
I just started playing with the editor 2 days ago and am having a blast! I have created a 5 level dungeon and have most of the floorplans completed. Granted creating the maps themselves seems to be the easy part. There are a few areas that I'll be adding to or changing, but I'll need to bone up a bit on my LUA first. The way you add new items to the game is kind of funky as you have to script them in but other than that this is a pretty good tool. Having the ability to playtest from the editor has saved me so much time in my trial and error method of learning the various functions. I just wish they had included a way to edit the character creator, I would really like to properly add some custom skills/traits/classes.
