Same issues. I immediately get an Icewind Dale has stopped working message. It won't open. I have Win 7 64 bit. Planescape and BG1 work no problem.
Run the config program and try enabling Windowed Mode (under graphics) to see if it makes a difference.

Also check the resolution setting to see if it's 'unsupported'.
Running as administrator doesn't help, nor does telling the machine to run in Windows XP SP3 compatibility mode. :(
The game also has an issue when you use the Creative Alchemy dsound.dll . When the dsound.dll is in my folder it simply crashes and burns and never starts. Thats pretty odd, works fine in BG1 and BG2 and even in Torment.. so yeah, thats odd. Would kinda like to have proper EAX in that game ;(

Also theres a goggame.dll which does not seem to serve any purpose whatsoever. But thats just random thing i noticed ;P
I can get the game running so it doesn't completely crash, but it forces me back to the desktop. The icon is still there to tell me its running. Every time I click on it the game runs for ~5 seconds then it pushes me back to the desktop. Run as Admin doesn't change anything.
I don't have the GOG version, but has anyone set Modify or Full permissions on the game directory and tried again? This won't fix any resolution problems, but may fix some other issues.
Just did a quick test on my Win7 64 bit rig. I installed as Admin and also installed the widescreen mod, the Unfinished Business mod, and the Tweak pack (all installed as Admin). I set the game to run in Win XP SP3 compatibility mode as well as to run as Admin. I firedn it up, made a character and got into the game. Went into and out of a couple buildings. Everything seems to work okay, albeit I've only tested for a few minutes. I'll report back if I run into any issues, but it may be a while.
My Problem Solved: I assigned the game to run as admin on every start-up. Then, if I just double-click the shortcut icon it still crashes to the desktop, but if I right-click the icon and select 'open' the game works fine. Strange, that's a new one for me.
If you use Firefox 3.6 and Vista X64 and have Firefox open and try to start games like Torment/BG/IWD they can not gain focus and maximize to fullscreen. So its pops up for 2 seconds (the video starts) and then goes back to minimized. Only Solution is to go back to a Firefox Version like 3.5.1 .. OR close the Browser entirely.

Running Windows Vista Home Premium SP 2, NVidia graphics card fully updated, Game crashes after creating new game full campaign to a black screen in upper left corner, I am the only user on the CPU running as administrator with now change...I DL'd twice and removed and reinstalled...still nothing. ~not happy~
A lot of users don't have any problems. But, I know, this isn't helping much I just want to say that it does run on both Vista and Win7.

that actualy worked for me... thanks. freaking black magic vodoo... im also forcing 6by4 and running as adi on all just to be on the safe side.

i had been playing for a while with no probs but then HoW was having trouble reading my "disk3" so i reinstalled it and then boom. insta crash.

funny how its been crapping out since my last big vista update... bumbumbuuummm
