Blame retailers, they won't carry AO games. They could carry them, if they wanted - but they don't. Since most retailers are national here in USA - yeah, they don't want that heat.
Back in the day, local gaming stores which were not chains here might've sold these kind of games - often behind the counter. RIP to many of those stores, BTW.
Other digital places sometimes sell AO versions, such as Direct2Drive, who sold Singles and Manhunt 2.
GOG even carries Fahrenheit as AO, even though basically that same version got a Remaster on Steam and that Remaster re-rated to M on Steam.
