wdluense3: Have you tried adding the d3d9.dll to the FO3 game folder? If not try the one provided in the fist link below. If that one does not work the next thing to try is downloading one from the second link until you can find one that works for you.

I tried them and nothing. I followed all the steps in the install instructions attachment for the first one too but no luck. I reinstalled like 3 times now. Thanks for trying to help though I appreciate it!

Would you mind giving your PC specs & which d3d9.dll version worked for you?
I have the Intel Integrated graphics card also, it "apprears" to only run relablity if I start the game through the fallout exe file rather than running it through gog galaxy. I did use the a dll file, but that alone didn't solve the problem.

No idea why. It is something to try anyway.
I had the same issue with the GOG version. Once installed, the game would load then crash after selecting new game due to the integrated graphics issue.

After inserting the dll fix, the game won’t get past the “please stand by” screen, and crashes to desktop. I tried running from the exe in the fallout3 directory, and game runs fine and can start new game with no crash.

So seemed to be an issue with launching in Galaxy, which is annoying.
I've just activated GOG version - Game crashes on Galaxy 2.0, but I tried what duke1313 did and game ran just fine. It seems like a Galaxy issue.
