Yes, but OP didn't ask for a list, he wanted people to give them one by each post. It's quite a nice exercise trying to remember all those quests.
Tuthrick: Yes, there are some that you didn't mention, here's another one:
Location High Hedge and the area to the south (Red Canyons) - bring Melicamp to Thalantyr, if the spell will be successfull you'll gain +1 to reputation.
gunman_: I found a list here (including negative reputation modifiers):

Edit: it seems to be missing some quests as well, like giving the angel skin ring to Fergus
I have noticed that Tales of the Sword Coast content has no reputation modifiers in any of the quests.
The list is a bit chaotic, but I've found another one that is missing from both:
Location northern part of Baldur's Gate city - kill Petrine's cat, then talk to Bheren. If your reputation is 1-7, you'll get +1 (similar to Cordyr and the sirines).
All quests from TotSC seem to be a little bit vague in terms of whether the outcome is pointing towards "good or evil", so there's no surprise there are no reputation modifiers:
1) Ice Island - killing all the trapped mages is no good deed, however they try to kill you first, so not evil as well (plus the reason why the party is being send there - either blamed for murdering Shandalar's daughters, or his selfish request).
2) Isle of Balduran - again killing most of the inhabitants, but saving the main land from Lycanthropy, very bittersweet ending.
3) Durlag's Tower - cleansing the tower, but cause the demon to be released (defeating the demon is rather correcting the previous mistake).
That's how I see it at least.
