It should be Vista and Win7 specific, only when installed to the default "Program Files", as that's the folder these two are real touchy about, and yes, it should also be game specific, as some games don't seem to run into permissions problems regardless.
But guys, this is mostly a guess based on the posts i saw around here about saves, as i always tick the 'Run as Admin' for every game to prevent permisssions problems that can mess with saves and also in some cases with settings (permisssions issues can prevent changes made to those settings from sticking), but i haven't put any of this to the test with Arcanum at my end.
I also assume that if the game is being ran as admin, saves will stay put even after unticking the run as admin on the shortcut, it's the other way around that might make them disappear.
I am having problems getting Arcanum to run in a windowed mode in Windows 7 (64bit.) Neither D3Dwindower or DXwnd will run Arcanum in windowed mode on my machine. Any suggestions?

Get the unofficial patch:

It fixes A TON of stuff, plus adds a windowed mode. To use it, just add "-window" to the game's shortcut.

I get occasional slowdowns while running the game, and I read in a post that -doublebuffer would fix that issue. However, after enabling it through the games shortcut, the game screen is shifted to the right (ie. the left third of the window is black, while the remaining 2/3 of the window displays game). I added -no3d for good measure, but the same issue still occurs. Any suggestions?
Note: I am running it in windowed mode on Wiindows 7 x64 professional. I also have the UAP, hi-res, high quality town maps patches installed

The long awaited patch for Divine Divinity has been released so it is going to be hard to get a response . . . =)
Your target path in the game shortcut should look like this -
"C:\Program files\GOG.com\Arcanum\Arcanum.exe" -no3d -doublebuffer
You should also uncheck the "Run this Program in compatibility mode for" on the Compatibility tab in the shortcut and check mark "Run this game as an administrator" at the bottom of the Compatibility page.
If this doesn't help and your path looks like the above example than remove the two switches and see if it reverts back to the normal appearance.
Also, if you entered the wrong resolutions or reversed the entries when installing the HR patch, you may have some problems. Usually you enter the screen size that is default for your computer. If you are seeing a smaller image with black you may have entered the wrong resolution or you have the "Run in 640 x 480 screen resolution" checked on the Compatibility tab for the game shortcut. If so, uncheck that setting. . . =)
Edit: Running in windowed mode has not worked well on Windows 7 for some reason so I would suggest playing full screen. Several have tried unsuccessfully IIRC to run in windowed mode.
If removing the switches does not fix your screen problem, either remove or reinstall the HR patch with the bat file found at C:\Program files\GOG.com\Arcanum\HighRes.bat Double check your default screen resolution prior to doing this so your entries will be correct. Also, when installing the game, mods, maps or the HR patch, right click the exe and click "Run this as an administrator" since Vista/Win 7 tend to block some disk writes if you simply double click the exe to install.

Thanks for the response. I should have mentioned earlier that the appearance is fine with both the doublebuffer and no3d switchesoff, but there are occasional slowdowns. These slowdowns do go away when I enable doublebuffer, however the screen ends up looking like the attached picture.

I believe removing the -window switch is the answer to your problem. Leave the -no3d and -doublebuffer switches.
You might send GOG support a help ticket on the issue. I would remove the -window switch and play on . . . =)
Hit F-10 to go to minimal UI . . . I really like to play that way . . . hit F-10 again to go back to normal UI when you need it.
Edit : your setting look like they should to me.
Edit 2: I added the -window switch on my XP Pro install . . . looks just like yours so . . .must be across all OS's. I had rather play full screen so I'm not affected . . .=)

Tuho: Runs fine i had some weird lags with inventory and looting, but just start it with either -no3d or -doublebuffer or both and that should clear it.
I use newest UAP patch and high ress & Hquality town mods scrolling seems little laggy with 1920 1200 ress but it didn't do it with the normal 800 600 ress.
I run windows 7 64

I really appreciate this, I used the -doublebuffer switch in the command line and it totally fixed my mouse issues with the game. Quick and easy, thanks :)
Win7 64-bit for me as well.
I have the old GOTY edition and am trying to run it with the UAP on Win7 (64-bit). From what I have seen, it is no different from the gog version. Yet, whenever i attempt to install the unofficial patch, I get an error message during installation saying that the UAP only works with the English version of the game. Has anyone else had this problem? Before you ask, i have installed the latest official patch and am running as administrator.

The game won't start for me. Screen goes black, I hear a sound start to play (very brief) then it switches back to windows with Arcanum still running in the taskbar. If I try to switch back to the game, it returns to windows again and eventually crashes the game.
Intel Core i7
ATI Radeon 5770
Windows 7 64bit
Game installed in G:\Arcanum.
I've installed the UAP and the Hi Res patch. I've tried every combination of -no3d and -doublebuffer and I've tried various combinations of Compatibility settings.
Any more suggestions?

Delete the Arcanum .cfg file and that should fix your crash....
It may also show up as an internet file with just the name Arcanum...
Should be one of the first ones after the folders in your main Arcanum install directory.

Also you shouldnt have to use any compatibility settings, so if it launches after you delete the .cfg Arcanum file put compatibility back to normal
Thanks Jay, I was having the same issue and that fixed in np. Just wanted to note my cfg file was called 'sierraup.cfg', so if you have a similar problem just trash that one and it should run fine.
