After just coming out of the cave at the very beginning, if you follow the stream until it becomes a miniwaterfall into the river, then go to the right of that spot, you can glitch off the side of the little cliff and fall into the water below.

Once this happens, there's no way out but to exit and load back in.

I also fell off the scafolding on the left side of town.
Did that fix it there too?
