After exploring the Stout Mines i find when i try to exit the guy at the door asks me for a Writ.
Well, i already gave him the only Writ i had when i entered.

I've went back down and have killed loads of dwarves but nobody has dropped a Writ, including King Freyedies himself.
What to do?
Easy to misplace things in a full inventory of scrolls, etc. So if he gave it back unidentified, that's the simplest solution. There's also the alternate, work-around item to give Pastollio, if you have any: Boogre Brew. Otherwise, he will just say to use the Writ on the Drawer. Quest items can be stolen (imps, brigands, nymphs, etc), so if he gave Writ back & it's missing, keep a lookout. Spirit Eye helps to spot it if it's on the ground somewhere after a steal, or if the PC inventory was full when Pastollio tried to return the writ. Best of luck!

He never gave it back.
I reloaded a save from just before i entered the mine, put the Writ in the drawer and he lets me in, but doesn't give it back.

You were right!
Strangely the Writ was in one of the equipment slots of one of my characters.

I'm inside the mines, and I never received the writ back when entering. Unfortunately, I didn't know you needed it to leave, so all of my saves are inside the mines. It's definitely not in my inventory (identified or otherwise). And no, I don't have the brew either. Am I just out of luck? I really don't want to restart the game after 30+ hours.

You could revert the game (from the inn), this should reset you to the last time you were in town.

Still lost time, but that saves starting over. You're a lifesaver!

Glad I could help. With this game, it's a good strategy to create a save-file before entering a dungeon and keep it until you finished it, just to have a fallback if things go very very wrong inside.
