I noticed some ppl had trouble streaming this game. This is what worked for me:

I use Open Broadcaster Software. I used the 32-bit version. I have Windows 7 Ultimate. I stream on Twitch. My desktop resolution is 1680x1050.

Game settings: I was able to stream the game when it was set to DX7, 800x600, 16-bit.
I was NOT able to stream properly when I changed the bit depth to 32, or Glide

Broadcaster settings:
You can stream either the game or the monitor. I recommend using the monitor.
Set the resolution to 800x600 (I assume that if you play the game in a different resolution that will work for you). If I set it to 1680x1050 streaming won't work. I *think* I got it working once but I couldn't replicate it.
Other than that using recommended settings from Twitch worked for me.

This way you can play the game in full-screen and still stream. The downside is that on my computer DX7 looks really bad while Glide looks really well. The I noticed this the astonishing change on Modai Textures.

In the first stages of my streaming experimenting I was able to stream my monitor output cut to the area where the game window was. This is dalso doable but I was really annoyed by the fact that my mouse went through the window edges.

Also note that you might have to start streaming before starting the game as Alt-Tabbing from game and back makes makes the keyboard unresponsive in-game.
