I am in Prague running through Josef's Tunnels. I have two companions, Wilhem and Serena. Wilhem follows me and (mostly) attacks the same enemy I am. Serena just stands still. I can switch to her and take control of her, but the moment I change back to Christof or Wilhem, she just stops and stands still, even if someone is attacking her. I have exited the game and reloaded my save, and she still simply stands around when not directly controlled.

Is there some way to fix her behavior, or am I stuck trying to dig up an older save from before the glitch?

Its been so long since I played that I can't remember the details but you can set each member to passive, normal, or aggressive stance (or some similar terminology) . I think there is a green dot to be clicked on or something like that. Sounds like she is in passive mode. Beware that on aggressive mode, they will attack anything - even bystanders in the street and will run off in places you likely don't want them. You can also set them to stay where they are if you wish (but they will show up if you move to a new area.
I've checked out her AI settings. It does not seem to be her being on defensive mode that is preventing her from following me, as she stays still even on the neutral or aggressive stance.

You might be stuck with an older save then - can't think of anything else that would cause that.
Sometimes party members will join you, but not be in the "group." Have you tried hitting the regroup button? Also check below their portrait to see if the group "light" is on. If not, try clicking it. I think pressing g or shift+g will toggle the grouping system.

This fixed all the problems. Even after restarting the game she was not joining the party, but simply hitting the regroup button once Serena instantly began following. Thanks a lot.

Glad to see that you're (Javokis) monitoring this now ; Won't have to quote answers you've made at VP anymore. :)
