I see the game has co-op as a listed feature, but I don't think the original game had a co-op campaign (unless you set up a dedicated server and did some other shenanigans, from what I researched). Does the GOG version have some mod pre-implemented that lets you play co-op, or what's the deal with it?

Serveral years ago I finished co-op game for Unreal and I don't remember any shenanigans in order to make it work and that was a disc version of the game. I started the multiplayer game with co-op game type and the map Vortex Rikers and was ready to go. I do remember however, that the Return to Na Pali expansion campaign had issues, because I couldn't get past the small intermission maps, where the player character narrates about what's happening, since they don't have exits. The workaround would be to skip to the next map, but then you lose all your weapons and items, so I decided it wasn't worth it.

So can you just select "campaign" and "co-op" and a friend joins you on LAN and that's it, or do you have to create a multiplayer game with a map from the campaign and play them in order?
You just have to start a multiplayer game with co-op game mode and select a starting map. I would go for the first map in the campaign - Vortex Rikers. Then the other players can join you and they will spawn in the other prison cells on the ship. From there you can go through the game as normal - if one of the players exits the map, all will spawn at the next one. Every time a player dies, he/she will respawn at the start of the current map.
