I see you are using Windows 7. Have you installed the game into the default Program Files folder? Well, Program Files (x86) being as you are running 64-bit. If so the it might be UAC/DEP getting in the way.

For game compatibility, especially older titles, I don't ever recommmend installing games into Program Files on Vista and Win7 as the Program Files folders are protected operating system folders and as such the OS doesn't like letting the contents of the folder be changed after the installation.

So, if this is the cause of the issue then you can try forcing the game to run as adminstrator (even if you are an admin account you still need to do this) by right-clicking on the GOG game launcher on your Start menu and selecting the "Run as administrator" option.

However, this said, I would re-install the game into your own games folder such as C:\Games\ so that it isn't located within a protected folder and running as admin shouldn't then be necessary.

As for patching, you shouldn't need to patch to 227h unless you want to (it is an unofficial patch). However, bear in mind that the DX10 renderer only works with the latest official patch which is what the GOG version comes with as standard. The DX10 renderer will not work with 227h (unfortunately, but maybe in the future a new version will work with it).
Its installed to C:/gog/Unrealgold/
Ran as admin
Tryed the direct x verision (did not patch it)
Reinstalled Tryed the patch version (same Problem)
Tryed every render Same problem

** CROSS POST HERE AND IN THE OTHER THREAD NAMED UNREAL GOLD ISSUE **

I've done a Google search on NALIFX.SHANEFX.TORCHES2 and it throws up a number of results for this issue. It also appears that the news is good on two fronts.

Firstly, it isn't GOGs fault, it is an issue with Unreal Gold itself that affects the retail version too.

Secondly, many people are giving the same simple fix. Turn off DEP (or add Unreal Gold to the DEP exclusion list).

Here are the sources I've used:
