So Exult is a popular program for running Ultima VII, but I'm wondering, is it needed? What advantages are there to running Exult over just running it in DosBox?

Running the game in DOSBox works just fine, that's the way it was meant to be played back in the day. However, that doesn’t mean it's the best way; Exult allows for higher resolution, more sound and graphics settings, smooth scrolling, inventory changes, mod support (like the keyring mod or the Exult rotate mod) and in general has a bunch of nice improvements. if you wan tto play Exult make sure you use the latest snapshot build, not the stable release. Snapshots are labeled "unstable" but they shouldn't be, plus the stable release has still the disappearing objets bug where things (items, NPC or even buildings) would just randomly disappear forever.

Thanks for that. I downloaded the latest snapshot build and everything is working great so far.
