I think it's what I've seen referred to as "flickering"; upon loading, the game flashes rapidly between the game and a black screen. It's worst on the cavedog intro and the main menu, it switches to flickering to a slightly darker version of the screen on map selection and in game.

Of the two solutions I've found online, neither have worked. Adding -d to the target in the shortcut doesn't make the game windowed or fix the flickering, and no messing around with vsync did anything either.

Any ideas what I'm doing wrong?

I had the same problem and solved it by changing the renderer. Open ChooseRenderer.exe in the install folder and try out the options, the Direct3D ones worked for me, but I have to play in a 4:3 resolution or it crashes when I try to start a battle.
