Can anyone give me any help. The game crashes at the first cutscene unless I put it in windowed mode. If I put it in windowed mode though i cant change the resolution unfortunately. Help!
Bump. Having the exact same issue.
I don't have the GOG version, I actually have an original TR1 disc from when it was first released, and I have similar problems to this, and I'm sure my problems are all DOS related. The thing that has worked best is downloading a Glidos package. The newest ones (google them) are not free and they are not the easiest things to get running but they DO work. You can probably find a more simple less expensive wrapper out there but I can't promise how good or bad they will look or work.

Glidos 1.52 has just been released. It should now work out of the box with the GOG and Steam versions.
I'm sure Glidos is a great piece of software, but I'm not interested in spending more money on a game I've already paid for. Maybe talk to gog.com about bundling it?

Anyhow, back on topic. I was having issues with the FMV in full screen hanging the game up, but now I can't even get to the main menu because the game hangs on the 3dfx splash screen. I haven't touched the game since having the issue with the FMV earlier. Once I finished Blood II I figured I'd come back to TR1 and try and see if there was a solution to be had, and ran into this new problem.

Gog.com is great, and they have amazing games, but I'd like to know if anyone has the Steam version of TR1 and what their experiences are with it?
It is really annoying. I am having this issue as well. As soon as the first cinematic loads (after menu screen) the game hangs and all i see is a black screen. If I start the game windowed it works without a problem. I tried something that worked (kinda).

On a moment of rage, when the screen went black, i windowed the program (alt+enter), and after a little while the game worked, but i couldn´t maximize the window again. After some fiddling with the config files, I set a custom resolution on the dosbox configuration and the glide ini, and on dosbox itself checked the option to unlock the mouse (ctrl+f10) and now i can maximize the window.

Also, i think is worth noting that when i selected a custom resolution "unsupported" by my monitor or a widescreen resolution, the game crashed without even entering the menu. So i left the resolution at 1024x768 and it works, although i have to minimize the window each time a cinematic plays. That is annoying, but at least i can play without problems after that.
I have the Steam version bought in a sale the other day for £9.99 (TR1 to TR Underworld) and TR1 works fine. I've changed the resolution to 1920x1080 and it hasn't had any problems. However, TR3 crashes when you click on New Game or Sound. I don't think it's so much to do with the GOG version or Steam version (I think the Steam version is the GOG version, actually; there's a file in the directory called gogwrap.exe) but more to do with typical PC imcompatibility problems. For the record, I have Windows 7 64-bit with an ATI 5800 series and an AMD Phenom II X4 3.2ghz.
@Export:

I didn't think to try TR1 on the only ATi/AMD machine I have. I've tried it on two different i5/i7 - nVidia boxes which result in the same issue on both. Guess I'll give it a go and see what happens.
I found another "solution" for my case (because minimizing was becoming tedious), although, you may not like it. Simply, don´t use hardware acceleration (not as pretty obviously). In your Tomb Raider 1 folder, go to TOMRAID, and change the name of "tomb" to something else (tomb3dfx maybe?) and the exe "tombno3dfx" rename it to "tomb".

Unfortunately, this returns you to the old pixelated software mode, but at least no more cutscenes/window problem. HOpe this helps a little.
Update.

Game ran perfect in full screen with resolution changes taking affect on my ATi/AMD computer. No hiccups at all. I guess I'll just play TR1 on the red headed step child and finish the series up on the good son.

To be honest, I don't think it's a nVidia-Intel / ATi-AMD yes or no senario. Seems to be luck of the draw for some.

At the rate this is going I should probably just play it in windowed mode.
I'm having the same issue with black screen on Win 7 64bit.
Any ideas how to get this running?
Right click on the shortcut and select Run as Administrator fixes it for me.

However the videos after the 3DFX logo doesn't seem to work (can hear audio but no video), and the main game looks like its rendered by software without 3D acceleration.
I got it running perfectly in full screen by installing nglide.
