For some strange reason. The game won't recognize my controls. And when I'm on the menu, the game just keeps moving out of control.

Update. When I try to exit out of it. it say Choosing pixel failed??

I am having a similar issue. I did get it to almost work, but then lara keeps trying to go backwards. I have no idea where it is coming from because it does this when a controller is not pluged in as well.
I'm having the same issue. the joystick on my controller can move the menu, but none of the keys will select anything. the arrows and "F" keys work on my keyboard but i can't select any menu items. The only way i can quit is to CTRL+ALT+DEL to get back to the desktop and right-click>close on the dosbox window.
I got two pads and both have same issue - menu keep spining around.
I'd disable the joystick altogether for DOSBox, it doesn't support enough buttons to play Tomb Raider anyway. I use XPadder to map the gamepad controls to the default keys and the game plays perfectly with them
I realize this puts me in a minority, but TR is my fave series ever and yet I can't play a single one of them with anything but a keyboard (keyboard + mouse for every one AFTER #5).

Just to follow up on this, for anyone confused by how to turn it off in dosbox.

Open the file Tomb Raider 1+2+3/Tomb Raider 1/dosboxTR.conf in notepad (Note: the one in the DOSBOX folder is not used for TR1).

Find the line joysticktype=auto, and change it to joysticktype=none.

This will make it so you can plug in your xbox 360 controller without it spinning at the main menu. Then just use Xpadder, or some other mapping program, to map the controller buttons to the keys you want them to be.

Seriously I happen to be on a TR/Kain binge and I've been going crazy plugging in and unplugging depending on what I feel like playing. Thank you!
For tomb Raider 1, I just found that it was the line "timed=true" (on [joystick] part), in the file on the path "Tomb Raider 1+2+3/Tomb Raider 1/dosboxTR.conf" wich caused the "crazy-menu-won't-stopping-turn" trouble for me (I changed it in "timed=false"). I precise it for GOG crew (if it interest them).
But I still had to use JoyToKey to use my gamepad (wasn't able to configure it properly ingame).

I did this as well
But instead of JoyToKey ,Dosbox seems to have an inbuild Keymapper that recognizes JoyPad Buttons
I just Alt+F10 in game then selected Key or Keyboard option
if you know which Keys do what select key to program ,just press "add" and press joypad button you want to use.
save and exit restart game and it worked for me

Sorry for the late reply. And this was a great solution!!!!!!!!!!!!
I solved the controls problem by adding the game to the STEAM LIBRARY and using the STEAM INPUT for generic controls, everything worked like a clock
