when i get to say the 4th level it goes onto the screen where it shows your results then after that it comes up with the cartoon screen showing a clip of a doctor doing something ... then it just goes back to the main menu ... HELP !!

It means you lost the level - you didn't meet the winning criteria, went bankrupt etc. You have to try again from a savegame or try restarting it.
