I can easily clear the first level every time, but as soon as I want to save level 2, it won't let me! This seems to be a thing with other GOG games I've bought as well including Under a Killing Moon, ie the save games don't work, and I can't save.

Any chance of patches?
I have the game on disc and I have the same problem
If you've used the default install directory, and you're running Windows 7, you'll get this - Windows 7 is very protective of the C;\Program Files and Program Files (x86) directories and basically disables write access to any folder in those directories. Older games usually had their save game folder in the install folder, more modern games use your user account to store their save games.

try installing the game to another directory (I use C:\Games so all my games are in one place) and see if that helps.
I'm bumping this as I am not using Vista (some time has passed since these posts lol), and I am still having the same issue, any ideas why?

Of course it is. That one for sure.
Another one is how you install it? GOG or GOG Galaxy? WIN or MAC?
This release use DOS+DOSBOX version. Saves should be ubder cloud_saves and SAVE folders but they wont available if game in program files. Normally VirtualStore sbould be used instead. But DoxBox game require to be installed - not extracted. It will set dosbox conf files to tell game which directories to use. And there is ingame hosp.cfg fike where installpath and savepath locations set after installing. Since it is dos virtualisation - it use c/.

So for GOG (offline version) issues can come cause of extracting instean of install (Linux maybe? But not big differences... Wait it is. Conf file path also should respect it is not NTFS system - and / \ // \\ and Capital letters are differ symbols)(for linux its about Win vers, if use Mac it already respect non-Win paths). Need set conf files.

For GOG Galaxy It is about Galaxy and games locations (D/Games/GOG/), admin rights to set up and configure game at first launch. For Galaxy version restart client choose Theme Hospital MORE Validate game files. It will also reset registry, links and dosbox conf files (and redownload some redists or something like that forbwhatever reason always did that).
Reinstall not equal to re-validate. During re-download somrthing can affect installation again and need validate at the end anyway.

What exactly do you want me to do?
Reinstall for GOG version or validate for GOG Galaxy version. Win or Mac release.
Ideally - reinstall to X/Games/Theme-Hospital and similar for Galaxy client
