When i try to add theme hospital to steam it just show me a DOSbox start client.

Why would you want it on that filthy scab of a place anyway? You have it DRM-free here, that works on modern systems. That's all that is needed :)

However, if you have bought it there and it doesn't work, it's probably better to contact those leeches rather than GOG.

Yeah, Steam is missing out some of the information from the shortcut so until Valve fix that you'll have to put it in manually for every dosbox game you add. It is possible, it just requires a bit of copying and pasting.
