Just found a bug that has supposedly been fixed some time ago. Currently playing Witcher 3 v1.31. Finished the main quest and currently exploring the content of Blood and Wine. While unlocking the mutations only one ability slot has been unlocked the other three are still locked while all mutations have been unlocked. Any solution?
