Hey guys,

since patch 1.21 the puddles look "chunky" (see attachment) . The newest patch 1.22 didn't solve the problem, but a rollback to patch 1.12 does. I tried different graphic settings or galaxy's repair tool. But the glitch still exists. Can anyone help me with this?

Maybe is this a video driver issue?
I have a similar problem with the textures of some bridges (in some points the textures gleams intermittently) and the terrain textures in Beauclair seem to overlap with a certain thickness, after I installed the last driver for my GTX 780.
I tried three different video drivers (GTX 950), but the problem still exists. Every time I started the game, game's water quality is also set to "low". I could find some other people with the same problem (since patch 1.21). I hope there is a workaround or something like that. These glitches are really bad (e.g. Novigrad).

I tried rolling back to earlier drivers for my GTX 970 and I still have the same exact problem as you. It's driving me crazy.
Yeah... and there are some other people with the same issues....

Have you found any solutions? Someone in the thread said they solved the problem by "loading the game files directly from gog.com" but I'm not exactly sure what that means.
I found a solution for myself. Hopefully it works for you as well. I have GOG so I don't know if you will have any success.

1. Download and install a program like Revo Uninstaller or IObit Uninstaller so that you can delete any residual files after uninstalling Witcher 3 (download and use the programs mentioned at your own risk).

2. Save your games saves elsewhere before completely uninstalling Witcher 3.

3. Completely uninstall Witcher 3 and GOG (or Steam if you have Steam).

4. Use one of the programs recommended in step 1 to delete any residual files left over from the Witcher 3 uninstall.

5. Restart computer (not a necessary step but it might help).

6. Reinstall GOG/Steam and then install Witcher 3 via GOG/Steam. In a little more detail, I redownloaded the GOG program from gog.com and installed it. Then I used the stand alone GOG program to download and reinstall Witcher 3 (including lastest patch and DLCs). Both GOG and WItcher were installed in different file paths for some reason so take this into consideration when you reinstall. In other words, try installing GOG/Steam and Witcher 3 in different file paths if installing in the previous file path where issues originated still produces the same problems.

Wow, this works :) I downloaded the full game + dlc's manually (gog.com). So right now I don't use GOG Galaxy or Steam to mange this game. Thanks for your support!!!

Will this solution work if I install W3 from a disc and install patches and DLC,s from gog.com? (in different paths of course)
I think you would be better served to redeem your Disc version code for a GOG key. The disc version is most likely the original the original release. Then download the latest installers, Patch(s), DLC package and Expansions.
