I have encountered two strange monster nests in Velen, which cannot be greyed out. They are next to each other in an underground cave. There is no nest where the map shows, only some "fog". I looted the whole cave and looked left and right, but they are still active. Is there any way to clear them?

You can clean the nests during a mission 'Wandering in the Dark' with Keira Metz. You will find her here when she is screaming. Kill the rats and their nest with a few quick casts of the Igni sign.

I already finished the mission (in fact, all missions), is there any way to clear it now? As far as I remember, I cleared them during that mission, but now it isnt greyed out.

The entrance to the cave is still open. Just go back in and work your way through to where you fought the rats for Keira. To clear the nests use Igni on them.

I did it, but there is no nest left where it should be. Just the empty floor, and a little fog on it. It seems to be a bug. I tried to throw bombs on the nest places, but nothing happened. I'll try Igni next time, thanks for he advice.
Yep, I remember now: I had to destroy these two rat nests during the "Wandering in the Dark" mission in the beginning of the game (as Jewo said). It was a mandatory step, so I sure did it, but now they aren't greyed out, and there are no nests either (just their "place"), so I cannot destroy it. Pity, these are the only ones missing...

What I find confusing on your map is that nothing is tagged as discovered -- not even Midcopse -- except for the entrance to Keira's cave (not the exit), and the two nests, but also a MUCH higher level area cavern, ie. Byways! This is very unusual.

It's not a screenshot from my game, I used an online map to highlight the problematic nests. :) I filtered out the rest to be readable.

Ah, I see. That explains it. I should have realised by the dimensions of the map. :)
I attached a screenshot of the problem.
