This isn't that big of a deal, but some trophies like "walk the path" (I just finished the entire game, with both expansions on death march), and "I have a gwent problem" are not showing up; the trophy one gets for obtaining aeroendight is also not showing up even though I clearly got the five virtues, and then obtained the sword...

Some achievements are bugged. You have to wait for a fix and do it again (load a savegame before the achievement and do it again).
One time a patch gave achievements you have done without doing them again. But your best chance is to keep a save, wait for a fix and do it again.
