I've only just purchased and installed The Witcher 3, so I am not very far at all. I was doing the tutorial fight in the beginning with Vessimir and got to the parry part. The on screen display tells me to use LT, I follow and Geralt doesn't raise his sword and I get berrated by Vessimir until I used the M/K. I got through the tutorial but I am wondering why my controller doesn't seem to work properly in this game.

I know its not the controller as I use it on Fallen Jedi with no issues. I checked the controller mappings and they are correct, it just doesn't seem to work in the game. Only a few of the actions work.

Any suggestions to how to fix this? Did anyone else encounter issues like these?

FYI - Its a wired Xbox one controller.
Make sure you plug the controller before starting the game.
Plug the controller and:
Windows Key + R -> joy.cpl -> test it
Windows Key + R -> devmgmt.msc -> Xbox One Controller -> Properties -> Update driver
Windows Key + R -> devmgmt.msc -> Xbox One Controller -> Properties -> Revert driver to an old one (2014)

Move xinput1_3.dll away from the folder. GOG Galaxy -> Witcher 3 -> Verify files.
You also can try to copy another xinput1_3.dll form another game.

Also disable Xboxapp for Witcher 3
