Is on max settings.

I even tried some of the tweaks that could help, nothing.

If anyone has any ideas, please share!

This mod provides you with more graphical settings in-game. There's one called something like decrease texture pop in. I used it in my playthrough but I'm not sure if it worked, nevertheless pop in was never an issue for me. It supports 1.12 so if you plan to play the Blood & Wine, you might want wait for an update to support 1.2 or going through the ini-giles manually and find something there.
