Make sure you are wearing the entire outfit (shoes, pants, jacket) and have completed all interactions with him. If you have done this I suggest you open your GOG Galaxy client (if you are using this) and use the verify/repair install function. I have heard of several people who experienced this bug and a repair should fix it.

Again I will suggest using the verify / repair function in the client and then replaying the last bit again. If this does not help I suggest removing all mods and trying it once more. Some mods (particularly those out of date) can cause such issues, but to me it sounds more like a corrupt and/or damaged install.
