I had finished the game and the save file was sitting there waiting for me to do new game +
instead of doing it i decided to fire the game and do the add-on, which i did not own at the time of finishing the game for the 2nd time (with new game + already )
i am afraid it messed something up for i am no longer able to do new game + even upon completing every single quest and even when i try to load the save prior starting the dlc quests, Halp !

New Game Plus *must* be started with a manual save (that you must do) in Kaer Morhen after completing the main game. If you don't have that save you will not be able to start NG+ .

i made myself a reason and i reloaded a save prior to finishing battle preparation or whatever this quest is named, then finished both add'on and then the main game after finishing it i landed in Kaer morhen and saved, the game told me i could now do the new game + so i tryed, to no avail, tell me can you only do new game + once ?

Yes. There is no NG++

That's a shame :( i could only get to level 86 at new game + end lol
