I've started my 4th playthrough of Witcher 3, this time in NG+. In my previous playthroughs when I did Keira's quest, I ended up having to battle her at the end of it and killing her. So, I come to my question: Which dialogue choices should I make to have her surrender the formulae without killing her?

I'm surprised you didn't make a save before you get to the tower, before you engage Keira in dialogue. That would have given you chances to reload from that save, if you didn't want to kill Keira, to try other dialogue choices that led to the "better" outcome, other than killing her or sending her to Radovid. As for exactly which dialogue choices, please google, as there are websites that have precisely the info you seek. (Not trying to be snide, I just don't remember *exactly* which choices you ought to make, lol. But I always google when I am stuck, and when I know I'm going to be meeting and speaking and probably making choices, I always save.)

You don't have to take the notes. Just offer her to go to Kaer Morhen. I forgot exactly what her line was, but, the message is something like, she hated living in the swamp and sleeping with bed bugs. Kaer Morhen should be one of the dialogue choice after that. She will show up during the Battle of Kaer Morhen. She will put the note to good use later on. Geralt will do an epilogue about that.
In essence, just don't be a jerk with her, try to understand the person she is, and when the opportunity arises (when she tells Geralt that she's going to Radovid), tell her that's suicide, and there's a better way: go to Kaer Morhen. Above all, do NOT be aggressive about the notes; those are her ticket out of the flea pit. Ultimately she uses them for good, so try to see that in her.
