I was fighting some Foglets in the middle of a swampy area and happened to fall into a hole from which there was no escape. The depth was such that it prevented Geralt from jumping, stating that "You cannot do that now," each time I tried. I tried rolling repeatedly all around the hole and there was simply no way out. I had no choice but to reload an earlier save. I just wanted to make CDPR aware of this so that it can be fixed and others may be saved from suffering this cruel fate. I couldn't get any screenshot evidence, but I'm certain Geralt's skin got all wrinkled, like it does when you stay in the bath too long!

I stepped into the hole while in combat, so I'm not sure if that might have had anything to do with why I could't get back out. Even after disposing of the Foglet from my compromised position there was still no way out.
