You can do the base game, then Hearts of stone, then Blood and wine.

To do the extensions, just do the associated quests.

NG+ lets you start a new game, but at the level and with the gear and chest content which you had at the end of the game. Your enemies will be of a higher level too.
To start the expansions, simply do their quests.

No idea if you can use the Origin installers on a GOG.com base game.
You do have to have the expansions installed. Once the base game is over, you can start an NG+ game which means starting from the beginning.
You can start the expansions once you have the level requirement for them. They have their own quest, with a different color text or exclamation mark. If you start a NG+, the level requirements for the expansions will also be higher.

You need to have both the games and the HoS and BaW expansions in stalled to play HoS and BaW.
There should be a mentioning that there´s a new quest availabe at the notice board at the 7 Cats Inn.
For HoS the level requirement is 32. Not sure of the level requirement for BaW, nor for HoS in NG+.

You can't purchase HoS and BaW here, if you don't have the base game here.

You could try to see if Judas or another blue is online and contact them through PM.

If you have both the base game and the DLC from GOG, it should work.
If you have the base game from somewhere else, you cannot buy the DLC at GOG.

If you have the base game from GOG and the DLC from somewhere else, the DLC may or may not work
If this is the case, contact support or a blue when they're online. They might be able to help you out. Especially if you can prove you bought the DLC elsewhere, by showing a receipt or something like that.
