I'm seeking technical advice on a plaguing issue with The Witcher 3. Cut scenes, as well merchant inventory screens cause crashes. Running the game currently on a GTX Titan. I recently upgraded from a Nvidia GTX 660 graphics cards. Will say, I had zero problems running the game. This was including since the release. I performed a clean driver install today, upgraded to the most recent drivers for my Titan. I've tried numerous settings via google searches, haven't been able to narrow down the issue. Cut scenes will crash while Hairworks is active and in boarderless window/fullscreen. It will also crash while hairworks is inactive and in fullscreen. Here's the crash reason: Crash reason: EXCEPTION_ACCESS_VIOLATION (3221225477), Error reading location 0xffffffff.

I am running two monitors. The primary HDMI monitor is being used for gaming. My secondary DVI is not. I'm rather frustrated by these technical issues. Further advice on the subject would be appreciated.

Something is causing a video memory error. You need to pin it down -- the easiest way would be to send CD Projekt a support ticket with crash dump info. However, there are some steps you can take:

* Disable computer security programs, like antivirus and DEP
* Tweak your nVidia control panel settings
* Roll back to a previous display driver

The game will continue to crash if you don't pin down what is causing the video memory malfunction.
Thank you for the response. I started the game up this morning, allowed it to sit for about 10 minutes not paused before entering a dialogue/cut scene. It did not crash, however. I'm unconvinced there's still no issue. I've sent a support ticket into CDProjekt as advised. Included a DxDiag report as well the crash log. I did notice something odd, within the DxDiag report. It registers the Titan as only having 4gigs of memory, yet it's a 6gig card. Unsure if that's a Windows 7 misread or what. Did some reading as well about rolling back the display driver on overclocked/superclocked video cards. I could very well try a roll back. Since, others have experienced issues with the newer Nvidia drivers and aforementioned video cards.
I have the exact same issue. Funnily enough, I have a Radeon R9 390, so it doesn't seem to be a graphics card issue.
I've pretty much installed TW3 right after the windows installation finishes.

Killing the gog galaxy client helps a little bit (it feels like it crashes less), but I still get occasionally crashes.
They all occur in cut scenes. Sometimes right at the beginning, sometimes along the cutscene.

Ran a few hardware checks, a graphics benchmark, everything seems to be fine...

The only thing I couldn't check was whether I have enough energy for my graphics card. Currently I am using a 550 Watt unit, which might be not enough. I have no clue how to check for that, though.
For about 40 hours I didn't encounter any stability problem and no crash.

Suddenly, after this time, the game started crashing to desktop at starting dialogues, randomly. The issue started to pop out after the in-game achievements notification was introduced.
It keeps happening even after turning the in-game overlay off.
Same for me but I have not installed the overlay, so cant be that!
Update 11/29/15

I've been meaning to provide an update on the issue. Instead of rolling back my drivers, I used EVGA Precision X to downclock the GPU clock and Mem Clock by -75. Cut scenes ran normal, I was able to test it through multiple cut scenes, 4 to 5 with no CTD's. Hopefully, this information helps for all who are experiencing issues with cut scene crashes. I wouldn't rule out driver rollbacks though, it may very well help you.
