Not sure if there's already a solution posted somewhere, can't be bothered to trawl through the forums looking for it. I seem to be stuck in Skellige, trying to fast travel back to meet Lambert in Novigrad upon near completion of "Following The Thread", instead goes to a cut scene and then closes. Any hints? Cheers.

Happened to me exactly like that from Skellige. I traveled to White Orchard, then back to Novigrad/Velen, never happened again after that, no matter where I traveled. Might not help at all but give it a shot.
