Sometimes in the midst of combat I notice the LMB to attack is unresponsive, or Q for casting acts strangely.

Just now with the current patch I was fighting the "special" nekker in the gaseous caves. Wasn't able to attack, so I had to spam Igni. For brief periods, I could not even do that. Other times I could cast, but not any special modes.

It's a known bug, has happened to me a few times. The only fix in my experience was to load a save game (always works) or reset controls/key mappings (mostly works but doesn't always work).
