Finished the main story about a week ago, took a break. Now trying to return after this new patch I cant play for more than 5 minutes before a crash. doesnt matter if its in the main menu or if ive loading into a save!?

I've checked all my drivers and everything is up to date. Stress tested my system on serveral other games/programs and no problems with any of those. Any help would be greatly appreciated

Are you using NVidia cards? please be more specific if you wish to address a problem, it helps no one to know exactly what your problem is dear.

If you are using NVidia cards, assuming you are, using 700 and 900 series, the GTA V ready drivers ver 350.12 are known to cause crashes, I've had many crashes with my 970 card until I've updated it to ver 353.06

Running i7-3770, 16gb ram, gtx 970. Driver version 353.06

No idea what is causing the problem but after five crashes, each 5 or so minutes apart. I was able to play for one hour without a problem

Check your crash log in

\The Witcher 3 Wild Hunt\bin\x64

Maybe this thread will help. Check Here

The thread mentions most of the issues causing this, so I will not repeat them. Also if you are using MSI Afterburner or the EVGA equivalent try running the game without it active.
