I have the Dun Banner book, the sword and the spearhead and now no entries in the journal. I tried to get thru the mist, but couldn't get to the otherside. Without the journal leads what's next?

Yeah, like bullsheet mentions, if you've obtained all the items, it's time to return back through the mist. It's a fairly straight-forward path (just keep running until you find the exit, don't stop to fight the wraiths as they are endlessly spawning). Pressing 'Z' will activate the medallion you were given by Dethmold, and you should see it move about, showing you the way if you get lost.

Tried the medallion but it does't work, plus the game froze up once and CTD once. The Dun Banner thing is a book and isn't listed in the quest section. The subquest Death Symbolized says go find the banner. Am I still missing something?

@ OP: It's a Dun Banner standard that you need for a Symbol, not a book.

That is SO true! Even for a witcher it is too bright - or it would be the same in the mines, which it is not. He should be able to see fine, but not like day , also one hour before and after midnight, there's actual SUN!

But the little surprises are still better by night and the waterfall area looks just splendid. :)
