You're supposed to 'fall' off, or do you mean you die?
If you're alive you're supposed to run up the piece of bridge that landed on it.

I had the exact same problem....fought with it for days.....someone from the site was kind enough to play my game save and get me past that point...and send it back to me...so I could continue. I think the problem..at least with my game was my old graphics card, and some sort of mouse glitch that they have yet to fix. But you are not alone!!! Please feel free to use my game save that is posted under "A game save from a desperate woman...same Kayran issue HELP"...that is the one that was sent back to me. You could also send a copy of your game save to the Witcher support techs and they will also do the same thing and get you past that point. Hope this helps!
I read something about temporarily lowering the resolution / switching to window mode for the fight.
But it's second-hand info, so I'm not sure if it works.
If you want to try it: report back about effects please.
I would suspect the game is having difficulties with your mouse. I have quite a fancy one and found that part to be really easy. A bit strange.

I have found though that when your CPU and GPU are working hard out, like in an intense battle scene, my mouse locks up for a bit also. Therefore, I think that the previous suggestion, which is a great idea, that lowering the graphics for that fight may do the trick.
