The one I regret the most is killing some innocents because of misclicks. A couple of militiamen in the Outskirts of Vizima, slashed during the clickfests that are the fights against barghests (I blame this on my lack of experience; The Witcher was the first action RPG I ever played). I was also guilty of the dead of one beggar in the entrance of the sewers. I had been slaying drowners in the sewers and, when I left, I saw this man standing in the platform next to the door. I didn't realize I still had my sword unsheathed, in my hand. When I naturally clicked on the poor man to talk to him, Geralt attacked him, killing him instantly. No one saw this one, I don't remember if there were any witnesses of the deaths of the militiamen, but in any case I didn't suffer any consequences for those. It would be cool if the NPCs were scared of you if you killed innocents at night.

I also accidentally had sex with Shani. I genuinely believed that the rose was a token of appreciation to Shani for being such a great party hosts. I should have learned by then when to expect a sexy moment between Geralt and a lady.

What other things you did in The Witcher 1 by accident or mistake?
Being put in a bad spot from accidently jumping cuz misclicks

Eating ingredients/quest items due to how small the inventory icons are because misclicks

Engaging with the wrong enemy due to how oddly large their clickboxes are because misclicks

Attacking a blue-named character that's assisting me in comba- oh come the fuck on, why is that even in the game?

Not saving often enough because...well the game just crashes a lot, but really it's probably me just misclicking an invisible QUIT GAME? YES panel.
I lost so much money gambling that I artificially increased the difficulty level.

I couldn't afford to upgrade to medium armour in Vizim, spent the majority of the game with the armour from the start of the game.

My luck will turn around in the second game right?

LOL. I tried that one.... " I also accidentally had sex with …" with my girlfriends but it only worked once.
