I've completed the quest Heat of the Day (saving Alina and using the wreath of Immortelles on Celina) and Ripples (making peace between the humans and vodyanoi), however, when given Aerondight, it doesn't update the quest Paths of Destiny and Berengar doesn't spawn no matter what I do, so I cannot complete Chapter IV.

Has anyone else gotten this? Is there a workaround or a fix? Thanks in advance.

That is weird. Have you talked to Berengar as much as possible before finishing Ripples? There's some shuttling back and forth between him and the Lady of the Lake that you might need to do first. Specifically, she talks about destiny and a witcher, and then you have to mention that to Berengar, and then bring his response back to the Lady. And there's also stuff about his involvement with Salamandra that you needed to talk to him about multiple times first.

I have talked to them as much as the game would allow. The quest Berengar's Secret has been advance to the point where it says that "He admitted that he worked with Salamandra and he should pay dearly..." and the Paths of Destiny quest says that "The Lady of the Lake promised me a gift. I need to be patient."

Ah... you get that gift by talking to the Lady of the Lake again. That's probably the holdup. I'm not sure if you need to do it before fighting Dagon or not. Hopefully you have a save from before that? I'm pretty sure I talked to her and got the gift before facing Dagon.
You don't get Aerondight until after you have fought Dagon. This plays out in three different ways, depending on how you settled the differences between the villagers and the vodyanoi.

A third quest that is interlocked with these two is "Alvin". I think you will need to have spoken to Alvin twice (once about destiny and his gift, then later about witchers).

Yes, I know that... but I also know that I was able to talk to the Lady of the Lake about her gift at one point, and it "solved" the Paths of Destiny quest. I can't remember if it was before or after I fought Dagon, but I think it was before.

That's a good point, Combined should make sure he's done that too.

I've talked to Alvin as much as possible (told him that we are the masters of our own destiny and that he should be a knight rather than a witcher). I've also exhausted all dialogue options available for Berengar, the Lady of the Lake and most other characters in Chapter IV, but nothing seems to help.

I realize now that I should have been more specific in my last post. There are actually two gifts that the Lady of the Lake gives you. She gives you Aerondight when you kill Dagon, but BEFORE that, provided that you chose the correct options in the "Paths of Destiny" quest, she will give you some talents you can use to buy abilities. The problem is, she says she will grant you a gift and the game tells you to come back later. So you have to either leave the island and then return, and talk to her again to receive the talents, or you might be able to do it by just meditating at one of the fireplaces on the island and then talking to her again.

So basically, before you fight Dagon, if you leave and return, and talk to the Lady again, there will be another conversation option. She'll grant you some talents, and the "Paths of Destiny" quest will be solved.

At least, that's how it worked for me. If you have a save from before you fought Dagon, load it and try it out and then let me know if it worked.

Here are some observations I made, after toying around with various saves.

Paths of Destiny doesn't update even if I receive the talent. It remains at the "I need to be patient" stage, no matter whether I receive Aerondight or the talent.

The talent is also received only after completing Ripples, which immediately initiates the Aerondight cutscene and leaving the Island to return at any point in the quest doesn't seem to help anything.

Completing Ripples before Heat of the Day also yields no positive results.

That is weird... I definitely received the Talent before killing Dagon, and I did it simply while talking to the Lady at her usual spot on Black Tern Island. And it definitely solved the Paths of Destiny quest.

I think that the unsolved Paths of Destiny quest is causing your problem, but I'm not sure how to fix it. It could be possible to do so by editing your savegame, but I don't know how to do that myself. I have seen Thiev do that for other people in other posts, so maybe you could try to get in touch and see if Thiev can repair your savegame here. It sounds like a scripting issue where things aren't triggering properly.

Just out of curiosity... if Berengar doesn't appear, does that mean you are unable to leave Black Tern Island?

Alright. Thank you for the advice.

I am able to leave Black Tern island, it's just that the world doesn't really change. All the quests are accomplished and nobody has anything new to say, and nothing really happens.

After the Lady of the Lake gives you Aerondight, she disappears from the game. So you will not find her; that is not a problem.

Berengar will not appear on the island until you attempt to return to your boat. Have you tried walking toward your boat?

Thanks .I found the way using numerous permutation and combination and loading game from chapter 4 start.
The one that worked for me was
all secondary quests of chapter 4>>took quest to kill dragon from lady of the lake and julian >>then saved the bride >>killed dagon>>told lady of the lake >>got the sword>>forgived Berenger then killing starts......

I have completed this legendary game after 10 hours marathon of chapter 4,5, and epilogue.
Loved the ending !! Now marching onto Witcher 2 ....

If all else fails I could provide you with a neutral savefile near the end of Chapter 4 or at the immediate beginning of chapter 5.
