The only quest where there is no neutral choice is "Gold Rush", the bank robbery. You must also persuade one of the aligned parties (Yaevinn or Siegfried) to stand with you in "The Unforgiven", at the end of Act III. But you can regain your neutrality later.

(You could say that you have no neutral choice between Triss and Shani, but that has no such significant consequences.)

i aligned with elves. Those fucking humans.. =) But that game.. it was lets say, there was not right or wrong.

MINOR SPOILERS:


You have to pick whether to give Alvin to Triss or Shani, but I don't think you actually need to get romantically involved with either of them if you don't want to. You have to specifically give a gift if you want to start a serious relationship. I haven't tried not giving the gift though, anyone know how much is changed if you do that?

I believe that some shops are also unavailable (the merchants choose not to sell to you because of your reputation after the choice).

yes but it doesn't mae a difference.
This isn't a result of chosing shani or triss
its a result from other choices.
Anyway its not difficult.

Or Kalkstein, if you choose the neutral path.
Some choices in the game (i.e. Triss vs. Shani) really have little to do with Geralt's factional neutrality, although they are no less interesting with respect to plot and some rather important story consequences.

However, for the factional conflicts:

In "Force Recon", one remains neutral by refusing to lead the Order assault against the elves. However, I received no negative consequences for completing this mission and was still able to be neutral later. During "Gold Rush", you have to choose sides to eliminate the possibility of helping one side in the "Free Elves" quest. If you help, say, the elves, in "Gold Rush" you have the option of helping the elves or remaining neutral in "Free Elves". Same thing if you choose to help the Order in "Gold Rush": you can help the Order or remain neutral in "Free Elves".

Inevitably, only "Free Elves" is the mission where you decide to be neutral or not, although if you are planning to side with the Order or with Yaevin, you have to choose their sides during "Gold Rush".

It is possible to come up with the wrong suspect during the investigation, but you will learn that Raymond is responsible at the end of the chapter regardless. Full details on the possibilities in the "Vizima Confidential" quest are here:
