In the Prologue I'm at the point where I have to deliver the potion to Triss upstairs, but there is a big hole in the staircase, so i can't go up. This is my second play through and I doesn't remember anything like this. As anybody else encountered this, or am i just missing something?

I think there is another staircase you can use. I remember being confused by this at first too.

The room Triss is in can be reached by going up the stairs and through the door to the corridor that lead you to the armory and where you obtained most the the ingredients for the potion to heal her. There is a room off the corridor with a staircase that leads to the room Triss is in.
