Try this:

Win7: Click the Start button, type "regedit" in the Search box and hit Enter.
WinXP: Click the Start button, then click on Run and enter "regedit" and hit Enter.

Navigate to HK_LOCAL_MACHINE->SOFTWARE->Surreal->Riot Engine. Delete SettingsDX105.
For 64bit users: Navigate to HK_LOCAL_MACHINE->SOFTWARE->WOW6432->Surreal->Riot Engine.

Next Run Suffering.exe and the config utility box will pop up. Under the Audio tab, uncheck the box next to Use 3D Sound Buffers.

Got it working. I just set comptability mode for Windows 95/98 and it worked fine and yes with the widescreen patch too!
I can't make it to work, there is no surreal entry in the registry. Very strange and annoying situation.
For anyone else trying to get this working on Windows XP just remove IndirectSound from the install directory by deleting all the dsound files.
