More precisely, what are your specs? You say "Windows 7", but which version of win 7.
Also, what's your graphic card, your processor, etc..

If you have an ATI card, did you change the default settings in Catalyst (under "Gaming/3D Application settings")? If so, try setting everything back to default, and if that doesn't work, simply deactivate stuff like Vsync, antialiasing, triple buffering, etc.

And before all of that, did you try running the game as Administrator? Also, if your game is installed in the "Program Files" folder, you should uninstall the game and reinstall it simply under C:\GOG.com\The Last Express.
Installing games in the Program Files folder is known to cause issues on Vista and Seven.

Hope this helps!
I don't think it's Windows related. I have Win 7 x64 and game works perfectly. Last time I finished it was few months back. Game itself runs with DOSBox, so you could try tweaking DOSBox settings or try different graphic modes (there's GOG's graphic mode setup .exe in game folder)
