I have searched for tutorials and it all uses windows and steam or just windows and gog but I use mac so I just dont know how :(

First, copy the Terraria.app to $HOME/Desktop.
Then download tModLoader v0.11.6.2 to e.g. $HOME/Downloads. Note the "How to Install or Update" instructions.
Extract tModLoader.Mac.v0.11.6.2.zip. This will contain:

I18N.West.dll
I18N.dll
README.txt
Terraria
libMonoPosixHelper.dylib
tModLoader
tModLoader-kick
tModLoader-mono
tModLoader.exe
tModLoaderInstaller.jar
tModLoaderServer
tModLoaderServer.exe

Highlight all of those files and copy them.
Then browse to $HOME/Desktop/Terraria.app/Contents/MacOS (e.g. Right-click - Show Package Contents Terraria on the Desktop) and paste them. Overwriting existing files.

The README.txt does note that you may need to add execute permissions once modded:

4. Run Terraria as normal. If it doesn't run, you might need to give execute permissions to 2 files. If so, open a terminal in the install directory and run the following 2 commands:
chmod +x tModLoaderServer
chmod +x Terraria

Thanks for posting your suggestion but unfortunately the way you suggest doesn't match what I can see/do on my mac Gog.com version of Terraria. In fact what is so frustrating is that the Github raises more confusion and questions than it answers. You at least have a detailed suggestion but unfortunately it doesn't work for me.

I'm with OP here. If it's possible and even easy to install tModLoader why aren't there clearer walk-throughs of how to do it?

It is puzzling that those lovely people who have created TML haven't provided a real-world triedandtested How-To.
